Supporting your health, wellbeing, and independence

The Wellbeing and Self Care Service supports patients registered with Stockport GP practices to improve their health and wellbeing.


We are a non-clinical service, offering practical, personalised support for issues that may be affecting your wellbeing. This could include things like managing long-term conditions, improving confidence and motivation, accessing community support, or addressing social or lifestyle challenges that impact your health.


We work closely with local GP practices to provide preventative support, helping people stay well, feel supported, and remain independent for as long as possible.
