Enhanced Pharmacy

Viaduct Care will provide a primary care Enhanced Pharmacy service to all of the registered patients of Stockport GP practices. The service model will improve the resilience in Stockport’s neighbourhoods by increasing capacity in primary care for the already overstretched GP’s by introducing an improved robust method of prescription management. Both clinical and non-clinical staff will support the repeat prescribing process, provide medication reviews and ensure improved drug monitoring. The service model is neighbourhood specific to consider the needs of local population cohorts within Stockport to improve equity and quality of care for all.

Having access to the Enhanced Pharmacy Team will assist all primary care providers by:

  • Providing support for repeat prescribing including improving the efficiency and safety of the systems and processes
  • Support with safe management of high risk drugs and improved systematic appropriate drug monitoring
  • Support with medication ordering and management for patients in all settings including in care homes
  • Undertaking high quality comprehensive medication reviews – concentrating on high-risk and vulnerable patients including those with mental health needs
  • Participate in National & Local initiatives
  • MDT support and guidance
  • Training and support
  • Providing lifestyle advice to patients and encouraging positive health & wellbeing
  • Promoting self-care & signposting patients to other support services
  • Supporting other services in the area including crisis response and find & prevent.
